Initial investigations said issues with the safety harness could have caused the deadly accident.
Investigators are reportedly probing if there was an issue with the harness, including whether it was properly fastened or checked before take off.
The Mayor’s office, however, suggested the fall “preliminarily occurred due to conditions”.
The instructor Emiliano Muñetón Castro was reportedly hospitalised after the accident.
