The Brazilian, who had been surfing at Playa Pavones, was impaled by the razor-sharp bill of the fast-moving fish.
The picturesque spot is famed among surfers for its long, rolling waves.
Fabiano, a PE teacher and canoe instructor from Itajai in southern Brazil, is now said to be in a serious but stable condition and is expected to survive.
His wife, Priscila Carlesso, revealed the shocking moment his life “changed in seconds”.
She said: “Fabiano went surfing and everything turned upside down – a needlefish pierced his heart.”
In a stroke of luck, a doctor who happened to be on the beach rushed to his aid and managed to stabilise him before emergency services arrived.
