After detaching from the track, the vehicle smashed into a shop window, crushing one person to death.
The victim was reportedly trapped under the trams wheels.
It is not yet known why the tram came off the tracks near the centre of the northern Italian city, a business hub currently hosting its Fashion Week.
Emergency medical services, including six ambulances and a medic car, scrambled to the scene.
Firefighters managed to extract passengers from inside the tram. Their conditions are currently unclear.
Civil protection teams have set up a tent to assist the injured.
Authorities have launched a probe to investigate the cause of the tragedy.
The incident involved a Tramlink tram, which can carry a maximum of 66 passengers.
Stretching to 25 metres long, the trams consist of three connecting carriages.
