Inspectors were checking tickets on the train when a row broke out with a 36-year-old German passenger.
Security were called to intervene after the German man, who was allegedly intoxicated, became aggressive.
But a violent struggle erupted between the passenger and one of the security workers.
During the scuffle, both men fell to the floor, authorities said.
Local media outlet BILD reported the assailant pushed the security worker against the door.
The force of the impact caused a piece of the entrance steps to break off, the outlet reports.
“At that point, for reasons yet to be determined, the door of the moving train opened,” authorities said.
Suddenly the 26-year-old fell out of the train which was moving at a terrifying speed of 74mph.

Emergency services rushed to the scene and launched a frantic search along the railway line.
The security worker, a Bulgarian national, was found beside the tracks around 1.25 miles from where the train stopped.
Police stormed the train and arrested the suspected attacker who had remained on board.
The public prosecutor has not revealed the man’s condition.
Investigators are now questioning witnesses and checking for video footage of the incident.
Examinations are underway on the train which has been impounded and is now being examined.
Cops are also investigating why the train driver did not receive a notification that a door had opened, which immediately initiates an emergency stop.
Employees of Germany’s national railway Deutsche Bahn have been repeatedly targeted by violent attacks recently.
It comes after a train conductor died after being assaulted by a passenger without a valid rail ticket in February.
Reports of the horror attack in western Rhineland-Palatinate sent shockwaves through the country.
The attacker was sentenced to 10 years in prison in July.
A court found that the man hit the conductor so hard he suffered a brain haemorrhage.
